Hospital Policies

 

By signing the registration form when registering any new pet with Friendship Springs Veterinary Care or when signing any subsequent consent form, I acknowledge my understanding, agreement, consent, authorization and acceptance of all the following policies for that pet.

 

General Policies

 

Ownership. Iassert that I am the owner or the duly authorized agent for the owner of the pet that I am registering, and that I am consenting to and authorizing the delivery and performance of services or products that I request in writing or orally including all additional ancillary procedures normally required in the delivery of that service or product, including but not limited to examinations, vaccinations,laboratory, imaging, pharmaceutical, medical, anesthesia, dentistry,surgical, hospitalization, boarding, or grooming procedures.

 

Guarantees. I understand and agree that neither Friendship Springs Veterinary Care, its agents, employees, doctors, nor subcontractors make or imply any guarantee or warranty of any kind whatsoever regarding the results or outcome of any diagnostic or therapeutic procedures,whether inpatient or outpatient, including but not limited to examinations, vaccinations, laboratory, imaging, pharmaceutical,medical, anesthesia, dentistry, surgical, hospitalization, boarding,or grooming procedures under any circumstance. I further understand that, while Friendship Springs Veterinary Care will do its best to prevent adverse events, in the delivery and performance of procedures such as noted above that sometimes adverse events up to and including death do sometimes occur. I agree that if I have concerns about any such procedures, risks,complications, recovery, post operative care needs, fees, or any other issues, then I will inquire about these concerns before the procedure has begun.

 

Definitions and Indemnification. Veterinary services shall mean all services provided by Friendship Springs Veterinary Care, its staff, and any subcontractors hired by same to perform such services. Such veterinary services include, but are not limited to, vaccination,medicine, surgery, dentistry, anesthesia, boarding, and grooming. All veterinary services, no matter how seemingly minor, have some degree of risk. Such risks include, but are not limited to, behavioral changes, injury, escape or death. Under no circumstance does Friendship Springs Veterinary Care, its staff, or subcontractors hired by same ever guarantee success or that complications will not occur from or during such services. While all reasonable precautions will be taken to prevent such complications from occurring, avoiding such complications is not always possible.Should such a complicating event occur with this animal/pet, I shall indemnify, defend and hold harmless Friendship Springs Veterinary Care, its staff, and subcontractors hired by same from and against claims, damages, losses and expenses, which may arise from such complications.

 

Hours of Staffing.  Employees of Friendship Springs Veterinary Care are notroutinely on the premises during our closed hours except for certain times that have been established for normal care of boarding and hospitalized pets. If this is a concern, I have asked the employees of Friendship Springs Veterinary Care to discuss my concerns and to arrange special care if needed and possible.

 

Dangerous Animals.  Friendship Springs Veterinary Care reserves the right to refuse to serve, see, admit, diagnose, or treat any animals that are deemed dangerous at our sole and professional discretion. This is without regard to whether or not we have seen the animal in the past since the past behavior of an animal is not always an accurate predictor of future behavior. Unfortunately, since we cannot always tell if an animal is going to be dangerous until after they have arrived in our facility, this may involve the pet owner and pet having made an appointment and trip to our facility already only to be refused service. Should such occur we do apologize for the pet owner's inconvenience. However, we will not be responsible for any financial or time losses to the pet owner for this inconvenience.This policy is for the safety of our employees, other clients, and their pets, and it is NOT subject to negotiation.

 

Abandonment. If this animal/pet should remain in the facilities of Friendship Springs Veterinary Care for a period in excess of five days beyond the time of scheduled dismissal, as determined by the sole discretion of Friendship Springs Veterinary Care, then this animal/pet shall be presumed abandoned. After this time has passed, Friendship Springs Veterinary Care will make one written attempt to contact me at the address I have provided to resolve this matter. If I have not resolved this issue to the satisfaction of Friendship Springs Veterinary Care within ten days of receipt of this letter, then I hereby acknowledge my abandonment of this animal/pet. Such satisfaction may require immediate payment in full of all services rendered and/or deposit in full for any additional requests for services.  In the event of my abandonment of this animal/pet, I hereby authorize Friendship Springs Veterinary Care to dispose of this animal/pet by whatever means it deems appropriate at its sole discretion. Such abandonment by myself and disposal of this animal/pet by Friendship Springs Veterinary Care does not alleviate my financial responsibility for fees incurred for requested services rendered plus any additional fees related to my abandonment of this animal/pet, for continued reasonable care for this animal/pet, and/or for fees related to collections of balances owed to Friendship Springs Veterinary Care.

 

Please note:  Should Friendship Springs Veterinary Care be required to send such a final notice of abandonment, the bill and/or any additional deposits may not be paid by check; satisfaction of the incurred debt may only be paid by cash, credit card, or Care Credit.

 

Extra-Label Drug Use. The general standard of practice in veterinary medicine includes frequent use of medications in "off-label"or "extra-label"fashion because of a paucity medications approved for pets. For this reason this is a practice tacitly approved by the FDA.Off label or extra-label means use of a drug in a manner not in accordance with the FDA-approved labeling for the medication. This includes, but is not limited to, use in different species, different doses, different frequencies, or for treatment of different problems than those originally intended. Friendship Springs Veterinary Care endeavors to follow the current standards prevalent in the veterinary community when using medications off-label or extra-label and in a slow a risk to pets as possible. I acknowledge the use of medications off-label or extra-label is a legitimate veterinary practice; that because specific testing may not have been performed by the drug manufacturer, then adverse events up to and including death may be ahigher risk for pets when using medications in this manner; if I have questions about specific medications in this regard, then I have asked them and they have been answered to my satisfaction; and with this knowledge I give consent to Friendship Springs Veterinary Care to use medications in off-label or extra-label fashion.

 

Euthanasia.  The policy of Friendship Springs Veterinary Care is to NOT euthanize animals that are considered to be healthy regardless of reason with the exception of animals deemed to be dangerous as determined at our sole and professional discretion.  This policy is not subject to negotiation. 

 

Fees Policies

 

Fee For Service. The staff of Friendship Springs Veterinary Care is always to happy to provide estimates for services. The only services included in the Medical Visit fee, Well Care fee, and similar service fees are the physical examination and/or consultation. All other procedures,services and items provided during your visit, no matter how small,most likely have an additional fee. If cost is a concern, I will ask what the associated fees are before I agree to accept any recommended service beyond physical examination. This policy is in consideration of the ever and frequently increasing fees veterinarians must face to be able to provide service.

 

Estimates.   An estimate is a rough calculation, or guess, as to expected cost of a procedure, but it is not a guarantee of final cost. The policy of Friendship Springs Veterinary Care is to provide estimates only, never quotes, for diagnosis or treatment of conditions or diseases. All estimates expire after two months, or if there is significant change in the condition for which the estimate has been provided, whichever comes first. Estimates will only be honored for sixty (60) days; after that time, you will need a new estimate which may or may not be the same as the first.

 

Because every patient is different, disease in every patient is different, treatment must be tailored to the individual patient, treatment requirements may change on a moment to moment basis, and the final outcome of the necessary treatments, as determined in the sole opinion of Friendship Springs Veterinary Care, is never completely predictable. Said another way there is no "cookie cutter" template to the treatment of specific diseases in specific patients. We will do our best to provide estimates, never quotes, that will be reasonably close to the final bill in a majority of cases, but there will always be cases that will have final costs that are lower or higher than the expected estimates. Therefore, we make no guarantees as to the final fees due.

 

 

Quotes. A quote is a guaranteed set fee for a procedure. The policy of Friendship Springs Veterinary Care is to provide quotes only for specific line items that have predictable and repeatable costs, and these items are specifically listed on the "Common Fees" page of this website. For all other services or items quotes are never, ever provided, including anesthetic, dental, or surgical time under any circumstance. All quotes for specific procedures expire after two months, or if the cost to Friendship Springs Veterinary Care to provide the service increases unexpectedly, which ever comes first.

 

 

If you are not certain whether you have received and estimate or a quote, please ask before service has been rendered!

 

Financial Responsibility. I accept full and total financial responsibility for any and all services rendered under the terms of these policies including services explicitly requested by myself or my designated agent as well as those services implied under the terms of these policies and such consents that I or my authorized agent have signed.These services include, but are not limited to, those services provided by Friendship Springs Veterinary Care, its staff, and subcontractors hired by same on behalf of my animal/pet.

 

Payment Policy. Payment for all services rendered are due either at the time of dismissal, when services are performed, or upon request and demand at any time. Friendship Springs Veterinary Care does not allow for billing, partial payment, or delayed payment unless specifically agreed to before services are rendered, and Friendship Springs Veterinary Care reserves the right to not grant such exception at any time for any reason. Pets will not be released from the hospital without payment or without prior payment arrangement. Friendship Springs Veterinary Care reserves the right to require partial or full deposits before services are rendered.

 

Forms of Payment Accepted. For your convenience, we accept MasterCard, Visa,Discover, AMEX, CareCredit, personal checks, and cash subject to the conditions noted below.

 

Check Acceptance. Friendship Springs Veterinary Care will only accept checks that receive approval from CrossCheck, a check verification system company. This allows us to minimize the risks of identity theft related to your checking account. If a check is declined, Friendship Springs Veterinary Care is not told why the check is declined, only that it is declined. Further information about why a check may have been declined is available from CrossCheck directly. In accordance with CrossCheck policies:

 

  • We will only accept checks from the person whose name is preprinted on the check,

 

  • The person presenting the check must be the same person whose name is printed on the check,

 

  • The person presenting the check must be present at the time of presentation with proper identification (driver's license);

 

  • We will not accept post dated checks under any circumstance;

 

  • All checks must be dated with the current date;
  • We will not accept any checks that have any dates, other than the current date, written on them anywhere including the memo line;

 

  • We will not accept checks that have any corrections, including “white out,” line-throughs, scratch-outs, or other corrections even if initialed;

 

  • We will not accept business checks, credit union checks, counter checks, traveler’s checks, out of state checks, or checks with multiple endorsements;

 

  • We will not accept checks from individuals who are not authorized signers on the account;

 

  • We will not accept checks with post office box addresses; that is, the preprinted information on the check must include a complete street address.

 

Also, we will charge a $35.00 fee for any check that is returned unpaid from the bank for any reason.

 

Credit Cards and Debit Cards. Friendship Springs Veterinary Care is happy to take these forms of payment for VISA and MasterCard, but they must be processed immediately, and the owner of the card must be present. We cannot hold credit card or debit card numbers to be processed at later dates.

 

Pet Insurance and CareCredit. Friendship Springs Veterinary Care strongly encourages its clients to consider application for Care Credit and/orpet insurance as a means to pay veterinary bills; this can be lifesaving in emergencies. CareCredit is a health care credit card with a credit line for treatments and procedures for your entire family, including your pets. It is designed to help keep other payment options, such as cash, Visa or MasterCard, free for other purposes. With many of the promotional offers, this service may also provide interest-free options. Please ask for more information.

 

Refunds. All sales for services rendered are final and not refundable. Refunds for dispensed pharmaceuticals may be final and not refundable,partially refundable, or fully refundable at the sole discretion of Friendship Springs Veterinary Care.

 

Admissions & Appointment Policies

 

Pet Restraint. For your protection, and that of others, all dogs must be on a leash and properly controlled while in the waiting area or exam rooms. All cats must be presented in an appropriate cat carrier or on a leash.

 

Appointments. To allow ample time for all patients and scheduled surgical procedures,while we do welcome walk-in patients, for your convenience were commend and encourage scheduling an appointment for your visit whenever possible. Medical emergencies shall always receive top priority, which is why an occasional appointment delay is inevitable. However, please realize that we will make every sincere attempt to see each client on time, and therefore regularly scheduled appointments receive priority over walk-in appointments,and that should your pet ever be the medical emergency, the same courtesy will be extended to you.

Drop-Off Appointments. For your convenience, drop-off' appointments are available. A 'drop-off' means you may bring your pet at the time that works best for you and leave him/her with us for a few hours. Usually we will ask you to drop off' sometime in the morning so our doctors can examine the patient in between appointments or at the time purposely reserved for admitted patients. Once the doctor is done,she will give you a call to go over the diagnosis and to give you discharge instructions.

 

Vaccine Requirements. All pets admitted to Friendship Springs Veterinary Care for treatment, boarding, or grooming are required to be "up-to-date"with their vaccines as noted below. The only exception is that certain sick pets may be exempted from this under certain circumstances and at our sole discretion. For dogs to be considered current on their vaccines, they must have been vaccinated for rabies,distemper, parvovirus, and bordetella in accordance with AAHA Guidelines. For cats to be considered current on their vaccines, they must have been vaccinated for rabies, herpesvirus, calicivirus, and panleukopenia in accordance with AAFP Guidelines. If my pet is not"up-to-date" as defined herein, or if I cannot provide proof of vaccination to that effect within one hour from the time of admissions, then my pet will be vaccinated at my expense without further ado. This is for the safety of the pet owner, the pet, other patients and their families, the staff and their families, the hospital and the community at large.

 

Parasite Control. Pets are not required to be on heartworm, flea, tick, or intestinal parasite control medications for admissions to Friendship Springs Veterinary Care. However, because most parasites are contagious to other animals, and because some of them are contagious to humans,if my pet is found to be infected or infested with any parasite while admitted to this facility, my pet will be treated accordingly at my expense without further ado. A partial list of common parasites for which my pet may be treated includes, but is not limited to, fleas,ticks, tapeworms, roundworms, hookworms, whipworms, coccidia, and giardia. This is for the safety of the pet owner, the pet, other patients and their families, the staff and their families, the hospital and the community at large.

 

Check-in/Check-out &Hospital. The hospital day begins and ends at 8 am. every day.We do not bill services based on portions of days; that is, patients are not charged an hourly rate for fluid therapy, nursing care, etc.The only exception to this is the "Hospitalization" fees which are billed in half day increments, from 8 am. to 8 pm. and 8pm. to 8 am. For all other services patients are either billed for the service or not. The phrases "one day" and "twenty-four hours" are often used synonymously; for our purposes at Friendship Springs Veterinary Care "one day" and"twenty-four hours" end at 8 am. every day regardless of whether a full day or twenty-four hours has passed or not.

 

Medications Refills

 .

From the Georgia Veterinary Practice Act: "It is unlawful for a veterinarian to release, prescribe, and/or dispense any prescription drugs without having examined the animal and established a valid veterinary/client/patient relationship."

Veterinary-Client-Patient Relationship means that: "(A) The licensed veterinarian or his or her licensed designee has assumed the responsibility for making medical judgments regarding the health of the animal and the need for medical treatment, and the client (owner or caretaker) has agreed to follow the instruction of the licensed veterinarian;

.

"(B) There is sufficient knowledge of the animal by the licensed veterinarian to initiate at least a general or preliminary diagnosis of the medical condition of the animal. This means that the licensed veterinarian has recently seen and is personally acquainted with the keeping and care of the animal by the virtue of examination of the animal or by medically appropriate and timely visits to the premises where the animal is kept; and

"(C) A licensed veterinarian is readily available for follow up in the case of adverse reactions or failure of the regimen of therapy."

There is no specific time designation that is stated in the Georgia Veterinary Practice Act as to what constitutes "recently seen and is personally acquainted with the keeping and care of the animal by the virtue of examination of the animal or by medically appropriate and timely visits."  This is left to the discretion of each individual veterinarian, ie based on his or her own best judgment.

Therefore, at Friendship Springs Veterinary Care, in order to meet the intended spirit of the Georgia Practice Act, we have deemed it prudent to require a brief medical visit every six months for renewal of all prescription medications except certain parasite preventive/treatment medications where we have extended the requirement to twelve months.  Additionally, in the case of heartworm medications we may require and current heartworm test, the timing of which may be based on the last dose of medication given.  If these requirements cause any inconvenience, we do apologize for that inconvenience.  However, we ask that you please understand that these requirements are based on almost thirty years of experience by our veterinarian and they are for the benefit and safety of your pet.  More importantly, there are based on law and they are not negotiable.
